• More comprehensive information about regular patients;
• Access to additional blended funding packages, tailored to patients’ health needs;
• Support for longer GP telehealth consultations with reduced administration costs;
• Provision of funding packages for general practices to provide comprehensive care to patients who are:
• frequent hospital users, and
• Australians in residential aged care.

General practices can register in the Organisation Register from 1 July 2023.  

• Registered patients will have access to longer MBS-funded telephone calls (Levels C and D) with their usual general practice from November 2023.
• Registered children under 16, pensioners and other Commonwealth concession card holders will have access to the triple bulk billing incentive for longer MBS telehealth consultations (Levels C, D and E) from November 2023.
• The new General Practice in Aged Care Incentive will support regular health assessments, care plans and monthly GP visits for people registered with MyMedicare in residential aged care homes from August 2024.
• Chronic Disease Management Plans will be linked non-exclusively from November 2024.

• 1 visit in the past 12 months for patients registered with Aboriginal Community Controlled Health Services and practices in MMM 6 & 7 locations
• 2 visits in 24 months for patients registered with Aboriginal Community Controlled Health Services and practices in MMM 1 - 5 locations 
• A parent/ child can be registered at the same practice, provided one of them has an existing registration
• Exemptions from any requirements apply to people who may be disadvantaged including people experiencing domestic and family violence, and homeless people.
