General Practice Support

WQPHN’s Primary Health Care Support program is dedicated to supporting the General Practice model of care including multidisciplinary care and integration across sectors and geographies. The program provides a wide range of support to general practices, clinics and services including general practitioners, practice employed nurses, practice managers and practice staff, locum doctors and students, allied health providers, Aboriginal & Torres Strait Islander health workers and other primary care providers to promote optimal clinical and health outcomes. 

WQPHN aims to improve the quality of care and practice systems by providing tools, information and assistance for creating a whole of practice team environment, chronic condition management (CCM), and the utilisation of MBS item numbers, practice incentive payments (PIP), workforce incentive payments (WIP), RACGP accreditation, systems management, immunisation, digital health, telehealth and specialised health and business management resources.

The Primary Health Care Support and Capacity Building Program aim is to:

  • Promote and support a General Practice based model of care
  • Support the adoption of best practice methods to improve quality of care
  • Provide access to quality tools to support quality improvement and data management initiatives
  • Assist practices in the understanding and meaningful use of Digital Health systems in order to streamline the flow of relevant patient information across the local health provider community
  • Support health information management to inform quality improvements in health care, specifically the collection and use of clinical data within practices
  • Support healthcare providers to navigate, set up and connection to My Health Record
  • Promote availability of refreshed training resources and access to face-to-face and online training
  • Respond to or coordinate requests for support and training
  • Provide advice to primary health care providers on changes in PIP and WIP incentives
  • Provide access and information about career pathways and education for primary health care providers and staff
  • Enhance and promote health pathways between providers, services and patients
  • Support can be provided via on-site practice visits, webinar, phone and email
  • On-site assistance can be one-on-one or in small groups at a time that suits the practice.

Primary Sense

Primary Sense is a population health management, clinical decision support, and data extraction tool, helping GPs deliver the right care to patients at the right time. Designed by GPs, data experts and researchers, Primary Sense extracts de-identified general practice data and uses evidence-based algorithms to provide GPs with real-time medication alerts, reports, and patient-care prompts. It also provides practices and Primary Health Networks (PHNs) with on-demand reporting to help
with population health management.

Benefits of Primary Sense for General Practice:

  • includes the Johns Hopkins ACG® System – the world’s leading population health analytics software – that identifies high risk groups and predicts future health needs to improve a patient’s care plan
  • is easy to use with most GPs and practice staff requiring little or no training to get started
  • provides real-time medication safety alerts, patient-care prompts and near real-time data extractions
  • provides on-demand reports to proactively plan care that can be used by GPs, Practice Managers and nurses
  • is compatible with major practice clinical software systems, such as Medical Director and Best Practice
  • is hosted in Primary Health Insights (PHI) with all extracted, de-identified data privacy protected, securely stored and managed in Australia.
  • Primary Sense is 100% funded by PHN’s for General Practices to use at no cost

Primary Sense provides Western Queensland PHN with a coordinated approach to supporting primary care, enabling high‑quality data extraction, better identification of service needs, and significantly enhanced analytics capability.

PHASES with Primary Sense

PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) is a statewide collaborative initiative of the 7 Queensland Primary Health Networks, Queensland Health and the Australian Government Department of Health, Disability and Ageing.  This initiative aims to reduce the burden of cardiovascular disease (CVD) by strengthening primary care in Queensland.

By leveraging the data analytics capability of Primary Sense, prevention efforts are strengthened, enabling a comprehensive review of patient needs at the point of care and giving general practice the opportunity to offer proactive guidance on lifestyle and behaviour modification alongside medication options.

The treatment of cardiovascular disease in primary care will subsequently improve long term health outcomes and reduce the demand and cost Queensland’s hospital and health services.
