WQPHN collects and holds personal information from individuals and organisations. We use that information to develop and support programs that will improve health outcomes for our community.

You can request a copy of information that we hold about you about you by completing the form below. We will need to verify your identity. If you are making the request on someone else’s behalf,  we will require evidence of your authority to act on their behalf.  Further information is available from the link below. ​​

Privacy Feedback ​

For individuals receiving health services who wish to make a privacy complaint should in the first raise privacy issues directly with the service provider.  ​​

​If they are unhappy with the response, they can escalate this to WQPHN via the following methods as outlined on our website Feedback Form.

​The feedback officer will manage feedback in accordance with the Complaints Handling Policy.  ​​

If an individual believes their privacy complaint has not been appropriately handled by the WQPHN, they should contact the Office of the Australian Information Commissioner 1300 363 992 (local call charge) or visit their website www.oaic.gov.au
