
Primary Sense is a population health management, clinical decision support, and data extraction tool, helping GPs deliver the right care to patients at the right time. Designed by GPs, data experts and researchers, Primary Sense extracts de-identified general practice data and uses evidence-based algorithms to provide GPs with real-time medication alerts, reports, and patient-care prompts. It also provides practices and Primary Health Networks (PHNs) with on-demand reporting to help with population health management.

PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) is a statewide collaborative initiative of the 7 Queensland Primary Health Networks, Queensland Health and the Australian Government Department of Health, Disability and Ageing. This initiative aims to reduce the burden of cardiovascular disease (CVD) by strengthening primary care in Queensland.
By leveraging the data analytics capability of Primary Sense, prevention efforts are strengthened, enabling a comprehensive review of patient needs at the point of care and giving general practice the opportunity to offer proactive guidance on lifestyle and behaviour modification alongside medication options.
The treatment of cardiovascular disease in primary care will subsequently improve long term health outcomes and reduce the demand and cost Queensland’s hospital and health services.
