Creating Greater Choices for At Home Palliative Care for all Western Queenslanders.
Being cared for and dying at home is most people’s preference. This project aims to help make that possible.
In 2017-2018, the Australian Government provided funding to 11 PHN trial sites to pilot the Greater Choice for At Home Palliative Care (GCfAHPC) Measure. The aim was to make it easier palliative care services. This supports people who have a life-limiting condition, by improving the choice and quality of care and support given at home.
In the 2021-2022 Budget, the Australian Government promised $37.3 million to expand the Greater Choice for At Home Palliative Care Measure to all PHNs in Australia, for four years, to 2024-2025.
The GCfAHPC Project by WQPHN works to enhance awareness and facilitate access to safe and quality palliative and end-of-life care within the comfort of individuals' homes. Our aim is to establish connections between education and training resources in palliative care throughout the Western Queensland region, fostering collaboration across all levels of health networks, including local communities.

In close partnership with local and state government networks, Hospital and Health Services, non-profit service providers, statewide and national palliative care projects and services, local businesses and community stakeholders, we strive to ensure a comprehensive and sustainable framework for enhanced care outcomes.
