Aftercare services provide psychosocial support that is trauma informed and delivered by partnership approach with the participant, empowering them to improve their safety and quality of life.
Participants are assigned a Peer Coordinator, who offers a range of supports, including but not limited to:

This service address low-intensity mental health concerns, whether it’s anxiety, health issues, work stress, relationship or family challenges, long-term isolation, loneliness, sleep deprivation, or other pressures.
LIME offers flexible and accessible support for people aged 12 and above, with five free sessions available by phone, or via video call.

Self-referral, Family/carer referral, Health professional referral, Community organisation referral, Local Aboriginal Medical Services or GP Referral.

GP referral
Children and young people between the ages of 5-18 (inclusive) at risk of a mental illness or have been diagnosed with a mild to moderate mental illness.
Linked to a school community.

At risk of a mental illness. Diagnosed with a mild to moderate mental illness. (Whilst Young People with a severe and persistent Mental Illness may be identified and engaged through this program, ongoing service delivery would be in collaboration with the local Child & Youth Mental Health Service (CYMHS) and Primary Health Care providers.)

Mt Isa Specific - Please note that Outback Futures is commissioned to deliver the WiSE program to the Mount Isa School of the Air students only.
For all other regions, the following referrals are accepted (Please note that all referrals must be with consent)
Outback Futures multi-disciplinary team provides a range of child and family, adult and relationship counselling and psychology supports through scheduled face-to-face community visits
and
Stay with Me phone/video support which offers regular and consistent therapy/support between face-to-face visits.
Outback Futures also responds to and supports individuals, families and communities effected by suicide.
There is no cost for Outback Futures core services and they do not require a doctor’s referral.

People who are struggling due to a tough period in their lives and need some guidance to improve their social and emotional wellbeing.
People who do not require the intervention pathways provided by primary mental health care services.

Support for:
Supporting people on their recovery journey and providing linkages to other services within a stepped care approach.
Can be accessed in the following towns and surrounding areas via telephone or face to face support: Winton, Longreach, Isisford, Ifracombe, Tambo, Blackall, Jericho, Alpha, Barcaldine, Aramac and Muttaburra.

Self-referral, Family/carer referral, Health professional referral, Community organisation referral, Local Aboriginal Medical Services or GP Referral.
People who have anxiety and/or depression or a diagnosed mental health illness – mild or moderate.
People who previously had a mental illness or have early symptoms of a mental illness and/or at a risk of developing a mental illness.
People with a mild or moderate mental illness and who are living in rural and remote areas and have barriers to accessing Medicare Benefits Schedule (MBS) based psychological intervention.
Also limited support for people with severe mental illness, or to people who have attempted, or are at risk of, suicide or self-harm where access to other services is not appropriate.

People living with a severe and complex mental illness and/ or substantially reduced psychosocial capacity due to the result of impairment caused by a mental health condition. Also are not yet engaged with NDIS or unable to receive support from the NDIS.
Ages – 16 to 65, (Under 16's WQPHN permission required).
The ability to purchase capacity building activities that will promote recovery, independence and social inclusion. Also, Individual support if established it is appropriate.

If any of these feelings are getting in the way of you living your life, then NewAccess can be a great starting point.
NewAccess Coaches are trained in Low-intensity Cognitive Behavioural Therapy by CBT Institue to help develop and guide participants through their tailored program. Low-intensity Cognitive Based Therapy is a structured, evidence based psychological treatment. Put simply, it allows us to recognise the way we think, act and feel.
NewAccess Coaches work alongside participants on a problem, develop an understanding of what is causing distress, and guide through the use of tools and strategies that can be used in day-to-day life.
