Service Details

Aftercare services provide psychosocial support that is trauma informed and delivered by partnership approach with the participant, empowering them to improve their safety and quality of life.

Participants are assigned a Peer Coordinator, who offers a range of supports, including but not limited to:

  • Supporting the development and maintenance of a safety and support plan tailored to the participant’s individual needs
  • Supporting participants to stay safe during a period of elevated risk and vulnerability
  • Providing consistent contact and building rapport
  • Facilitating access to a range of community support services (e.g. relationship, housing, employment, Rural Financial Counselling, or education supports)
  • Encouraging uptake of hospital discharge plans and safety plan utilisation
  • Supporting attendance at critical follow-up appointments
  • Reducing barriers to the participant accessing related follow-up care
  • Assisting participants to connect with support networks
  • Providing or referring the participant to emotional and/or cultural support

Service Details

Provide culturally and clinically integrated social and emotional well-being service, within a person-centred stepped care approach, which is accessible, affordable, culturally responsive to Aboriginal and Torres Strait Islander Peoples.

Also targeting Aboriginal and Torres Strait Islander Peoples with co-morbidity conditions - mental health and chronic disease.

Support provided by brief interventions, counselling, initiating and managing planned treatment options, consultation and referral to other agencies or crisis management.

Mental Health Nurses in Practice for people with severe and complex mental illness

Client Need

People with severe mental illness being managed in primary care, including clinical care coordination for people with severe and complex mental illness through the phased implementation of primary mental health care packages and the use of mental health nurses

Ages – all ages

Service Details

Clinical care coordination using a single multiagency care plan managed by GPs and psychiatrists.

Other service provision includes:

  • monitoring a client’s mental state;
  • liaising closely with family and carers as appropriate;
  • administering and monitoring compliance with medication;
  • providing information on physical health care, as required and, where appropriate, assist in addressing the physical health inequities of people with mental illness; and
  • improving links to other health professionals/clinical service providers.

Delivered by face to face sessions, as well as telephone and internet-based services.

Low Intensity Mental Empowerment (LIME) Program

Client Need

This service address low-intensity mental health concerns, whether it’s anxiety, health issues, work stress, relationship or family challenges, long-term isolation, loneliness, sleep deprivation, or other pressures.

LIME offers flexible and accessible support for people aged 12 and above, with five free sessions available by phone, or via video call.

Outback Futures

Wellbeing in Schools early intervention PROJECT (WISe)

Client Need

Children and young people between the ages of 5-18 (inclusive) at risk of a mental illness or have been diagnosed with a mild to moderate mental illness.

Linked to a school community.  

Ages – 5 to 18

Service Details

At risk of a mental illness. Diagnosed with a mild to moderate mental illness.  (Whilst Young People with a severe and persistent Mental Illness may be identified and engaged through this program, ongoing service delivery would be in collaboration with the local Child & Youth Mental Health Service (CYMHS) and Primary Health Care providers.)

  • Initial Mental Health assessment of the Young Person.
  • Provision of a brief report outlining the recommendations for ongoing management.
  • Triage against the WQPHN Mental Health Stepped Care approach with facilitated referral to most appropriate management option.
    • This may include facilitating a referral to the Young Person’s GP for consideration of a Mental Health Treatment Plan (MHTP) and
      • ongoing management through the General Practice including options of utilising a telehealth psychiatry service.
    • Limited brief intervention service delivery with the Young Person and/or their family/significant others (up to six sessions per financial year, budget dependent).
    • Joint development of management plans with the school community and other key stakeholders.
    • Provision of culturally appropriate service delivery.

Mt Isa Specific - Please note that Outback Futures is commissioned to deliver the WiSE program to the Mount Isa School of the Air students only.

For all other regions, the following referrals are accepted (Please note that all referrals must be with consent)

  • Attending school and referred through their school community or GP.
  • Not attending school with self-referral or any key stakeholder with consent, including Young People suspended from their school community.
  • This may include facilitating a referral to the Young Persons GP for consideration of a MHTP and ongoing management

Outback Futures

Mental Health and Wellbeing Program

Client Need

Outback Futures multi-disciplinary team provides  a range of child and family, adult and relationship counselling and psychology supports through scheduled face-to-face community visits

and

Stay with Me phone/video support which offers regular and consistent therapy/support between face-to-face visits.

Outback Futures also responds to and supports individuals, families and communities effected by suicide.

Service Details

There is no cost for Outback Futures core services and they do not require a doctor’s referral.

Low Intensity Mental Health Services

Client Need

People who are struggling due to a tough period in their lives and need some guidance to improve their social and emotional wellbeing.

People who do not require the intervention pathways provided by primary mental health care services.

Support for:

  • work stress or uncertainty
  • change in living arrangements
  • new parent worries
  • family problems
  • health concerns or uncertainty
  • long-term isolation or loneliness
  • financial worries

Supporting people on their recovery journey and providing linkages to other services within a stepped care approach.

Can be accessed in the following towns and surrounding areas via telephone or face to face support: Winton, Longreach, Isisford, Ifracombe, Tambo, Blackall, Jericho, Alpha, Barcaldine, Aramac and Muttaburra.

RFDS Longreach

Psychological Therapies for Rural and Remote, Under-serviced and/or Hard to Reach Groups.

Client Need

People who have anxiety and/or depression or a diagnosed mental health illness – mild or moderate.

People who previously had a mental illness or have early symptoms of a mental illness and/or at a risk of developing a mental illness.

People with a mild or moderate mental illness and who are living in rural and remote areas and have barriers to accessing Medicare Benefits Schedule (MBS) based psychological intervention.

Also limited support for people with severe mental illness, or to people who have attempted, or are at risk of, suicide or self-harm where access to other services is not appropriate.

Commonwealth Psychosocial Support Program - Brokerage

Client Need

People living with a severe and complex mental illness and/ or  substantially reduced psychosocial capacity due to the result of impairment caused by a mental health condition. Also are not yet engaged with NDIS or unable to receive support from the NDIS.

Ages – 16 to 65, (Under 16's WQPHN permission required).

Service Details

The ability to purchase capacity building activities that will promote recovery, independence and social inclusion. Also, Individual support if established it is appropriate.

Key areas of focus could include capacity-building in one or more of the following areas:

  • Social skills and friendships;
  • Family connections;
  • Managing daily living needs;
  • Financial management and budgeting;
  • Finding and maintaining a home;
  • Vocational skills and goals, including volunteering;
  • Educational and training goals;
  • Maintaining physical wellbeing,
  • including exercise;
  • Managing drug and alcohol addictions, including tobacco;
  • Building broader life skills, including confidence and resilience.

Developed by Beyond Blue, NewAccess is a free mental health coaching program for anyone who is feeling stressed, anxious or overwhelmed about everyday life issues such as work, study, relationships, health and/or loneliness. If any of these feelings are getting in the way of you living your life, then NewAccess can be a great starting point.

NewAccess Coaches are trained in Low-intensity Cognitive Behavioural Therapy by CBT Institue to help develop and guide participants through their tailored program. Low-intensity Cognitive Based Therapy is a structured, evidence based psychological treatment. Put simply, it allows us to recognise the way we think, act and feel.

NewAccess Coaches work alongside participants on a problem, develop an understanding of what is causing distress, and guide through the use of tools and strategies that can be used in day-to-day life.
