WQPHN has commissioned Geriatric Care Australia (GCA) to deliver initiatives aligned to the Ageing in the Outback Strategy and Implementation Plan, including:
- Geriatrician consultation and care planning
- Delivery of Healthy Ageing education for consumers and workforce, including physical activity, frailty awareness, chronic disease management, understanding complex geriatric syndromes and social prescribing
This partnership focuses on improving access to timely geriatric care, strengthening workforce capability, and supporting proactive screening and early intervention for older people, particularly in rural and remote communities.
RN-Led Virtual Care Coordination Model with Geriatric Care Australia
WQPHN has also commissioned GCA to deliver an RN-led virtual triage and care coordination model pilot program.
The model includes:
- RN-led virtual triage
- GP oversight
- Proactive preventative reviews
- Geriatrician escalation as required
- A value-based healthcare approach
In 2026, the program will be piloted in areas of need across Western Queensland.
